This position is eligible for Eskaton's health benefits package the first of the month following 60 days of employment and Paid Time Off benefits program after 90 days of employment. Position Summary: The starting salary for this position ranges from $21.75 to $28.14/hour. Factors such as scope and responsibilities of the position, candidate's work experience, education/training, job-related skills, internal peer equity, as well as market and business considerations may influence base pay offered. Position Responsibilities include: * Identifies psycho-social needs of Assisted Living, Skilled Nursing Facility, and Memory Care residents through evaluations made at the time of admission to facility and upon staff/physician referral. Makes appropriate referrals to community agencies (i.e., adult protective services). * Documents adherence to the plan of care for social services and updates the plan as changes occur. Assists in the scheduling of resident care conferences and participates in conferences as appropriate. * Maintains current community resource information that may be required for residents at time of admission or discharge. * Participates in utilization review as necessary. * Completes all psycho-social assessments within seven days of resident admission to the Skilled Nursing facility and two weeks for the Assisted Living Unit and the Memory Care Unit. * Makes recommendations for room changes for residents needing to move due to psycho-social problems. * Works in conjunction with the other departments to meet resident's needs. * Note taking at IDT meetings. * Conducts community tours as necessary. Qualifications Education: Training and Experience: Job Knowledge: The final candidate must successfully pass Eskaton's post offer, pre-employment testing which includes a criminal background check, drug test, COVID test, TB screen test and health screen.
