Summary:
Provides full range of age appropriate discharge planning services for assigned area in the hospital consistent with the health system and departmental mission and goals. Coordinates community support systems enabling the patient to return home or facilitates relocation of patient to an extended care facility. Provides crisis intervention and consultation on emotional, social, and financial aspects of patient's care. Acts as integral member of the interdisciplinary health care team.

Responsibilities:
1. Intervention Techniques-Identifies patients requiring Social Work intervention through established case finding mechanisms.
2. Patient Analysis-Gathers and assesses biopsychosocial data on the patient and family.
3. Continuing Care & Discharge Planning-Identifies/communicates psychosocial and discharge planning needs of the patient upon referral and throughout hospitalization; Sets priorities in planning/organizing timely interventions, continuing care and discharge plan based on patient, family, community resources, and health care team information.
4. Intervention Implementation-Implements appropriate interventions based on patient and family, cultural, religious, and ethnic beliefs.
5. Patient & Family Education-Provides patient and family education as needed and evaluates response to information/interventions provided.
6. Communication-Communicates effectively with patient, families, health care team members, and community providers
7. Positive Relationships-Maintains cooperative working relationship with patient, family, health care team members, and community providers.
8. Documentation-Documents pertinent patient data in the medical record; Documents Social Work interventions related to patient placement including patient/family response, problems, and required action as needed but at least every seven days.
9. Cooperation-Seeks assistance of Director, Medical & Risk Services when needed in assessing complex patient/family situations and consulting physicians.
10. Teamwork & Participation-Attends and participates in department staff meetings; Assist with orientation of undergraduate Social Work students, staff, or volunteers; Participates in quality improvement activities as requested; Attends continuing education programs (100% completion of mandatory programs).Reporting-Submits departmental statistical reports accurately and timely.Professionalism-Upholds a professional working relationship at all times and adheres to department rules. Practices teamwork and adheres to Standards of Behavior.

* Bachelor's degree in an appropriate discipline or an equivalent combination of education and experience.
Licensure/Certification Requirements:
Professional Experience Requirements:
* If a Bachelor's degree, requires three (3) years of related experience.
* If an Associate's degree, Requires seven (7) years of related experience.
* If a High School degree, Requires eleven (11) years of related experience.
Knowledge/Skills/and Abilities Requirements:
* Understands and applies principles, procedures, requirements, regulations, and policies related to specialized expertise.
