Health Registry Assistant Internal Only

Summary

Under the direction of the provider or designee, this person is responsible for maintaining the patient population health registry data for the assigned providers. This position identifies care needs and works with providers to ensure delivery of excellent health care to our patients. The population health database is used to identifying care gaps, chronic disease and well health, using evidence-based guidelines. With the gaps in care data, the specialist will work with the providers and practice manager or site supervisor to encourage excellent patient communication, closing gaps in care identified by evidence-based guidelines to increase compliance and wellness. With the gaps in care data the specialist will work with the providers and practice manager to assure the patient (s) have appropriate test/visits performed.
REQUIRED CERTIFICATION AND LICENSURE
AART- Reg Radiologic Technologist
CMA or RMA- Certified Medical Assistant or Registered Medical Assistant
EMT-S: EMT specialist


  • Equivalent Experience: As required by Medicare and Medicaid Services (CMS) in order to have clinical access in the Electronic Medical Record, prefer one of the following active certification/licensure: Registered Medical Assistant (RMA) through American Medical Technologist (AMT) Certified Medical Assistant (CMA) through American Association of Medical Assistants (AAMA) OR equivalent Medical Assistant Credential such as (Certified Clinical Medical Assistant CCMA, etc.). Medical First Responder, Emergency Medical Tech Basic/Specialist, or Paramedic though the State of Michigan. Registered Radiologic Technologist through American Registry of Radiologic Technologists (ARRT)


REQUIRED EDUCATION
Education: High School Dipoloma or GED


  • Comments: Knowledge of all scheduling procedures and protocols normally acquired through two years experience working in a medical practice or similar setting is required. Experience in a healthcare setting is preferred


ESSENTIAL DUTIES AND RESPONSIBILITIES:


  • Under the direction of the provider or designee this person is responsible for maintaining the patient population health registry data for the assigned providers. The specialist will work with that provider to encourage excellent patient communication about closing gaps in care identified by evidence based guidelines to increase compliance and wellness.
  • Targets patients for intervention based on evidence based guidelines, initiates communication through mail or portal correspondence regarding appointments, health maintenance and needed care to ensure all gaps in care are addressed.
  • Prepares report information from the population health record and tracks summary information on care interventions.


EXPERIENCE, TRAINING AND SKILLS


  • Knowledge of all scheduling procedures and protocols normally acquired through two (2) years' experience working in a medical practice or similar setting.
  • Experience in a healthcare system preferred. Working knowledge of PC software to include Microsoft Office with aptitude to learn MyMichigan Health programs (Epic, Lawson, Allscripts, Misys, Cerner, and Kronos).
  • Excellent interpersonal skills needed to communicate successfully with individuals and interact with people at all levels to communicate concepts in a clear and understandable manner.
  • Mandatory Occupational Safety and Health Administration (OSHA) training must be done annually according to the Exposure Control Plan.
