Description

Penn Mar Human Services is looking for a Quality Coordinator with a passion to serve and work within the community to provide superior care nd support for Adults with Intellectual and Development Disabilities.

As a Quality Coordinator you must have the ability to implement quality case management, person-centered planning and discovery techniques to help people you support connect their interests, skills and conditions for success with short and long-term goals that will enable them to reach their "good life." Ability to work with each person on your caseload to develop individualized schedules that reflect their needs, desires and goals. Knowledgeable about the entire Customized Employment process and facilitate the discovery process.

Work collaboratively with support team managers to ensure quality individualized activities are accessible to each person. Ability to communicate thoughts clearly, maintain proper documentation, and problem solve. Maintain open communication and cooperative work efforts with employees, families, and community stakeholders to ensure quality services. Demonstrate clear and concise written communication. Demonstrate advanced knowledge of Person-Centered planning approaches, Discovery, and communication. Demonstrate professional behavior, make appropriate decisions, and be a role model.

Responsibilities:

  1. Maintain a caseload of ~25-30 people.
  2. Facilitate the planning process for individuals using the case management "roadmap" to support people to identify and achieve their goals.
  3. Support people through the Customized Employment process resulting in competitive independent employment opportunities.
  4. Responsible for all case management activities. This includes ensuring that all aspects of PCPs, funding, etc. are completed for each person in a timely manner.
  5. Actively work with people to develop goals and assist them to maintain and improve levels of skill and/or independence. Develop person centered outcomes and goals and train the team on implementation procedures related to each person's goals.
  6. Support people on your caseload to create individualized schedules based on connecting with people, places and activities of importance to them in their community. Schedules should be reviewed routinely with the person to access relevance to goals.
  7. Demonstrate understanding of Mandt training and behavior as a form of communication. Strategize with the person and their team to develop effective approaches and document them.
  8. Maintain the master file for each person on your caseload in the data management system. Review information annually. Update information as it is received.
  9. Review daily logs for people on your caseload at least monthly and provide feedback to the support teams. Communicate progress with the person and their team.
  10. Conduct monthly check-in's to ensure quality of services; this may include modeling and/or support for the team.
  11. Aid people and their families in understanding their funding and connect them with additional funding and/or resources to achieve their goals.
  12. Provide other case management duties as needed. This may include but is not limited to coordination and collaboration with primary caregiver/residential provider, assistance with obtaining and maintaining entitlements and resources, and supporting someone to manage their medical appointments and medications. Review and update medical information at least annually.
  13. Maintain continuous contact with support teams and job developers to communicate pertinent information related to the care of people on their caseloads. Consultation with the job developers will be required to aid someone in accessing independent employment
  14. Maintain continuous contact with caregivers and Service Coordinators to communicate pertinent information related to the care of people on their caseloads.
  15. Participate in Quality Assurance by facilitating the investigation and reporting of incident reports. This includes obtaining additional follow-up information from related stakeholders and communicating incidents to required stakeholders (as indicated in the IR protocol). Log and track all incidents in the State's reporting database within the 24-hour reporting window (occasionally reporting is required during evenings and weekends). Participate in QA meetings as needed.
  16. Participate in the Admissions process by attending school IEPs, coordinating informational sessions clearly conveying the service delivery model and vision. Gather paperwork as part of the Admissions process and provide recommendations to the admissions committee. This responsibility will be spread across the Quality Coordination team and assigned by the Quality Coordination Manager.
  17. Build effective relationships with families, caregivers, residential agencies and resource coordinators.
  18. Attend/Participate in QC meetings and other meetings as assigned.
  19. A mixture of office-based and community-based work environment; some home visits required
  20. Complete necessary training and certifications to perform job duties
  21. Participate in the hiring process for staff within the quality coordination team as directed by the Quality Coordination Manager
  22. Demonstrate knowledge of, and compliance to state and agency regulations and requirements. Be able to articulate these to people and their families.
  23. Complete reports/documents as assigned in a timely, concise, accurate and understandable manner (payroll, incident reports, CIMS, PCPs and tools, assessments, etc.
Requirements

Qualifications:

  • Bachelor's degree preferred; 3 years of experience in the Human Services field required.
  • Must have valid driver's license and no more than 2 current points against license.
  • Demonstrates advanced knowledge of daily living skills, communication, community interaction, and recreation.
  • Must be flexible and able to work independently
  • Mast have a current valid US Driver's License for at least 2 consecutive years ( Driving time while holding a Learner's Permit does not qualify).
  • Must have less than 3 moving violations (NO Major Violations, D.U.I's, Suspensions) in the previous 36 month.
Salary Description
$19 -$23 an hour
